Less alcohol is the lower-risk choice, not a guarantee against illness. New Zealand's low-risk drinking limits do not mean risk-free drinking. 1
For perimenopause, the useful question is not whether alcohol is simply healthy or unhealthy. Consider breast cancer, sleep and bone separately, and distinguish observed associations from proven benefits.
Start with the unit
In Aotearoa New Zealand, one standard drink contains 10 g of pure alcohol. A poured glass is not automatically one standard drink. Check the label and the amount you actually pour. 1
For example, if a bottle contains eight standard drinks and you pour a quarter of it, that serving contains two. Counting glasses alone can hide the difference between a small serving and a generous pour.
NZ guidance for women advises no more than two standard drinks a day, no more than ten a week, and at least two alcohol-free days each week. These are limits, not targets. 1
The separate limit of four drinks on an occasion addresses injury risk. It does not replace the lower daily guidance for long-term health. Medicines and health conditions can also make drinking inappropriate. 1
Breast cancer risk rises with intake
Sun and colleagues pooled 22 prospective cohort studies containing 45,350 breast cancer cases. They found a linear dose-response association for total alcohol intake. 2
For postmenopausal women, each additional 10 g daily was associated with an 11.1% higher relative risk: risk ratio 1.11, with a 95% confidence interval of 1.09 to 1.13. 2
This concerns average intake over time, not the effect of one isolated drink. The association was mainly evident for oestrogen-receptor-positive breast cancer. 2
Relative risk is not percentage-point risk. As a mathematical illustration, an 11% relative increase would change a hypothetical 3% risk to 3.33%, not 14%. That is not your personal forecast.
The estimate is postmenopausal, not a perimenopause-specific calculation. A fitted linear trend does not establish an exact effect at every tiny dose or a risk-free allowance. 2
Ask your doctor about personal risk rather than applying this population estimate to yourself.
Alcohol changes REM sleep
Sedation and healthy sleep architecture are different outcomes. Gardiner's 2025 review of 27 studies found delayed REM onset and reduced REM duration. 3
Its dose models estimated different points for different outcomes. Using the paper's illustrative 62 kg bodyweight and 10 g drinks: 3

These are model estimates, not personal thresholds or recommended doses. Effects on total sleep time and awakenings were uncertain; timing and sex-specific effects were not resolved. This was not a menopause-specific review. 3
The practical conclusion is to avoid treating a nightcap as a sleep remedy, rather than assuming sleep is unaffected below two drinks.
Hot flushes are a separate question. The Menopause Society identifies limited evidence for trigger avoidance, including alcohol, as a treatment. Improvement is possible for an individual, but cannot be promised. 4
Bone evidence depends on the outcome
Bone mineral density, hip fractures and all fractures are not interchangeable outcomes. The studies do not support giving all of them one J-shaped label. 5 6
Godos's review included 11 studies with 46,916 people for bone density and eight with 240,871 people for fractures. Lower intake was associated with higher density at some sites. 5
In the paper's reported drink units, up to two daily was associated with higher lumbar-spine and femoral-neck density, and up to one with higher hip density. 5
Hip-fracture risk ratios were 1.33 at three drinks daily and 1.59 at four, compared with no drinking. Their confidence intervals were 1.04 to 1.69 and 1.23 to 2.05 respectively. 5
These are the review's drink categories, not verified NZ standard-drink equivalents. They should not become a personal threshold below which fractures cannot occur. 5
Ke's larger review included 44 articles, 6,069,770 participants and 205,284 fractures. Total fracture risk rose linearly by 6% per additional 14 g daily, with a 95% confidence interval of 2% to 10%. 6
However, hip and osteoporotic fractures showed J-shaped associations, with lower observed risks at intakes up to about 22 g daily. That is 2.2 NZ standard drinks, not a recommended intake. 1 6
These observational findings cannot prove that alcohol protects bone. Differences between drinkers and non-drinkers, including age and activity, complicate interpretation. Higher density is not proof of fewer fractures. 5
The balanced conclusion is neither that every low intake damages bone nor that low intake is protective. Total fracture findings also prevent a blanket claim that low intake is harmless. 5 6
Wine is not a heart-health strategy
Biddinger and colleagues studied 371,463 UK Biobank participants. Light-to-moderate drinkers tended to have healthier lifestyles; adjusting for these differences weakened apparent cardiovascular protection. 7
Their genetic analyses linked higher alcohol exposure with hypertension and coronary artery disease. Modelled increases were smaller at light intake and steeper at heavier intake. 7
Genetic analysis can help investigate causation, but depends on assumptions about the genetic variants used. It is not a trial assigning people to drink, and does not precisely forecast your response to one glass. 7
This evidence challenges the claim that wine should be consumed for heart health. You do not need to start drinking to pursue cardiovascular prevention. 7
Make the trade-off explicit
Choose a question you can act on: how much am I drinking, why am I drinking, and what happens when I change the pattern?
Before trying alcohol-free days, seek medical advice if you drink heavily or have experienced withdrawal. Cutting down can itself trigger withdrawal symptoms. 8
Otherwise, a practical starting plan is:
- Record standard drinks, drinking times and occasions for a typical week. Use labels rather than counting glasses. 1
- Choose alcohol-free occasions, smaller servings or lower-alcohol alternatives. Tell someone supportive what you are trying. 9
- Keep a simple sleep diary: bedtime, awakenings, morning alertness and night sweats. Compare your usual pattern with a period of reduced drinking.
- Treat any symptom change as personal feedback, not proof about your cancer risk or bone density.
You do not have to decide your lifelong relationship with alcohol in one evening. Start with a clear, manageable change rather than searching for a supposedly perfect number.

Shaking, nausea or a racing pulse after reducing alcohol need medical advice. Seizures, collapse or other severe withdrawal symptoms require emergency care. Do not try to manage these through coaching. 8
If hot flushes or night sweats remain disruptive, discuss treatment options with your clinician rather than relying on alcohol reduction alone. 4
